This form is for FLC faculty to submit tests or other timed evaluations of student learning to DSPS for test proctoring due to extended testing time accommodations. You should have received notification from DSPS that a student with an approved extended testing time accommodation has scheduled a test proctoring appointment with DSPS. Please complete this form at least one day before the scheduled appointment date.
